ROSEBURG – While thoughts of going back to school might be on the back burner, the Umpqua Health Alliance has announced dates for local athletes to get ready for the upcoming sports year.

The Umpqua Health Alliance is offering students of the Roseburg and Glide school districts a chance to fulfill their required sports physicals. Adolescent sports physicals will be offered for Roseburg middle and high school students at the Roseburg High School Health Center August 1-5 and again on August 8. The physicals will be offered from 9 a.m. to 6 p.m. Those interested can make an appointment by contacting UHA at 541-492-2055 or walk-ins are also welcome.

Student-athletes in the Glide School District can fulfill their sports physical requirements August 8-10 at the Umpqua Community Health Center in Glide. The doors will be open from 8 a.m. to 5 p.m. each day and appointments can be made by calling 541-496-3504 or walk-ins are also welcome.

Student-Athletes must bring the sports physical form included in the athletic department sports packet and have it signed by a parent or guardian.

Physicals are also available for students within the Roseburg and Glide school districts that do not participate in athletics.
